In keeping with our mission to "develop innovative service models,"
The Boston Home initiated Bridges, Innovative Services for Adults
with Disabilities. The program enables the staff at The Boston Home
to reach beyond our doors and respond to the needs of adults with p
hysical disabilities--primarily multiple sclerosis and related
neurological diseases.
Under the direction of The Boston Home's Care Management Coordinator
Norma Harrington, RN, LSW, MSCN, Bridges provides comprehensive
services to help adults with physical disabilities live as
independently as possible. Bridges' services include:
information and referral, in-home evaluations to help in
accessing care, home adaptation evaluation, assistive
technology consultation, family support services, advocacy,
care coordination, and placement coordination.
The Bridges program is an example of how The Boston Home,
established in 1881, uses innovative services to preserve
its traditional values that foster independence, personal
dignity, and self-determination.
